Primary Insurance
We are contracted with many health plans, including Blue Cross and Blue Shield of Colorado, HMO Colorado, Rocky Mountain HMO, Medicare and Medicaid (Colorado), to accept their allowed amount. We will bill these plans directly, and will require you to pay only the authorized co-payment and deductible.
Co-payments are due at the time of service.
We anticipate periodic additions and deletions to our list of contracted health plans. Please contact our office or check this web site for current information regarding our participation status.
If you have insurance coverage through a plan with which we do not currently participate, we will collect from you any amounts not paid by your insurance. We will prepare and send your insurance company a claim. The insurance company may send the payment to you or to us. If it is sent to you, full payment on your account balance must be made immediately on receipt of the insurance check. In the event your health plan determines a service to be "not covered," you will be responsible for the complete charge.
If your insurance requires a referral from a primary care provider, it is your responsibility to obtain one.
Secondary Insurance
Our office will submit secondary insurance claims for you.
Discounts
If you do not have insurance coverage through which you are eligible for a discounted contracted rate we offer a 20% discount off of billed charges when payment in full is received at the time of service.
Self Pay
If you do not carry health insurance payment in full is due and expected at the time of service.
Payment Plans
We are pleased to offer several payment plans, including a 3 month "no interest" plan and extended payment plans ranging in length from 24 to 60 months, through CareCredit . You can conveniently apply for a CareCredit payment plan online, by telephone at 800.677.0718 or at our office.
Late Cancellation Fees
Digestive Health Associates charges a fee for visits or procedures that are not cancelled within 24 hours of the scheduled appointment.

- Procedure visit $100 (Southwest Endoscopy or Mercy Regional Medical Center)
Collection Service Charge
Overdue accounts are subject to a $25 collection service charge, which is assessed at the time the account is referred for collection agency action.
